Plain-language disclosure for the diagnostic form and any related inquiry.

What we collect

The diagnostic form at /book-diagnostic collects the four fields a senior leader needs to provide for a credible first read: name, email address, company / role, and a free-text answer to "what keeps you up at night about execution" (capped at 4,000 characters). Standard request metadata (user agent and referrer) is also captured at the moment of submission for operational troubleshooting.

How we route it

Inquiries are forwarded to Andrea through the platform email proxy that powers this site. By default messages route to andrea@eqfive.se; in deployments where the site is operated on behalf of a different owner this can be overridden via the ANDREA_EMAIL environment variable. The submission also registers a contact record and sends the respondent a single acknowledgement email confirming receipt — nothing else is sent automatically. No third-party CRM, marketing, or analytics tooling is wired to this form.

Retention

Each inquiry is stored in the site's diagnostic_inquiries table and retained for the duration of the diagnostic-practice relationship — i.e. as long as there is an open or recent engagement with the submitter. Diagnostic records are not used for marketing, retargeting, or any automated outreach. Records can be deleted on request at any time, as outlined below.

Your rights and removal

To request access, correction, or deletion of any data you submitted through the diagnostic form, email andrea@eqfive.se. Removal requests are honored promptly — typically within five business days — and confirmed by reply. There is no charge for exercising these rights.
